Initial Consultation

Service Description

You will be sent a comprehensive questionnaire to fill out prior to our scheduled visit regarding health history, symptoms, diet, lifestyle, lab work, and goals. I will review this before hand and during our time together we will address goals and prioritize based on symptoms. I will make personalized recommendations on diet, lifestyle, recommended further lab testing, and lay out of general time frame for you based on your goals and symptoms. I will educate you on some of the major things we can work on first.
